NHS NEW MEDICINE SERVICE

ARE YOU LIVING WITH A LONG-TERM CONDITION AND TAKING A NEW, RECENTLY PRESCRIBED MEDICATION?

This service provides support to people who are newly prescribed a medicine to manage a long-term condition, which will generally help them to appropriately improve their medication adherence and enhance self-management of their condition.  

THE SERVICE IS SPLIT INTO 3 STAGES WHICH ARE:

PATIENT ENGAGEMENT
Following the prescribing of a new medicine, patients will be recruited to the service via a referral from their GP or our Pharmacy team.

The new medicine will be dispensed as usual, any counselling provided, and the patient will be offered the opportunity to sign up. Following consent, the pharmacy staff and patient will agree a method and time for the Intervention stage, typically between 7-14 days after patient engagement. 

INTERVENTION
This discussion will take place either face-to-face in our consultation room or alternatively via telephone. We will assess your adherence to the medicine(s), identify problems and determine the need for further information and support.

FOLLOW UP
The last stage of the service takes place typically between 14-21 days following the intervention. We will provide advice and further support where necessary. If a problem is identified, we can mutually agree on a solution but were deemed necessary we will refer the matter to your prescriber.
